In this second JBJS/VJO video supplement to his article, Dr. Mininder Kocher of Boston Children's Hospital discusses ACL reconstruction issues in the adolescent patient, specifically Tanner 3 with substantial growth remaining. The surgical procedure presented involves a transphyseal autograph reconstruction with the hamstrings, femoral fixation utilizing an endobutton and tibial fixation with an interference screw. Both femoral and tibial fixation points are kept away from the growth plates. This physeal sparing reconstruction provides excellent outcomes with a low revision rate and minimal risk of growth disturbance.
