Suppose a patient presents an arthritic (painful) first MP joint and does not want a fusion? Furthermore, you are aware that studies show that silicone implants are not an effective solution for the long term.

The need for such a procedure, to resolve a painful arthritic first MP joint that was beyond a cheilectomy, motivated Dr. William Hamilton to develop the capsular interposition arthroplasty procedure.

The technique, demonstrated in detail in this segment, involves the reattachment of the extensor hood to the flexor hallucis brevis and minimal bone resection of the proximal phalanx.

This procedure, presented at the American Orthopaedic Foot and Ankle Society, won the prestigious Roger A. Mann, M.D. Award.
