Femoral Lengthening Over Nail

Dror Paley, M.D., of Maryland, reviews his surgical procedure for gradual limb lengthening with a reamed intramedullary nail. In a recent matched control comparison, the results of patients who underwent femoral lengthening over nails (LON) were compared to those who underwent gradual mechanical distraction with an external fixator alone. The outcome of this study confirms that the advantages of LON include reduction in external fixation treatment time, protection against refracture and earlier rehabilitation.

While technically more demanding, the most unique part of Dr. Paley's LON technique involves a procedure not commonly found in other techniques, namely, the utilization of a cannulated drill to place the external fixation pins. The use of this technique and the exceedingly apparent benefits are reviewed in detail.
