Mobile-bearing knee designs represent an alternative to conventional fixed-bearing TKR. These implants potentially offer a substantial increase in articulation conformity, with a beneficial distribution of the proximal and distal contact stresses and a decrease in the potential for polyethylene wear. However, even with these apparent biomechanical advantages over fixed-bearing designs, the use of mobile-bearing knee implants has remained relatively rare in the U.S., especially when compared with that in other countries. The issue of FDA regulation, the suspected technical difficulty of implantation and the cost of the devices all have been factors that have impeded their use. In this JBJS/VJO supplement to the article, Doctors Sorrells and Greenwald discuss these issues and Dr. Sorrells provides surgical video detailing his technique.
