Proximal Row Carpectomy with Partial Capitate Resection

Material Covered: The partial capitate resection modification of the proximal row carpectomy (PRC) has evolved over two decades. In a recent series, orthopaedic surgeons at Roosevelt Hospital in New York have found that satisfactory motion and relief of pain can be obtained when a PRC includes partial resection of the capitate, even in the presence of normal cartilage. Therefore, X-ray findings of degenerative changes at the lunocapitate and radiolunate joint need not contraindicate a PRC. By creating a simple broad pseudarthrosis between the radius and the distal carpal row, a functional, generally pain-free wrist pseudarthrosis can be achieved.
In this VJO segment, Dr. Eaton demonstrates the modified PRC, discussing the surgical technique and its implications. He says that this modified PRC procedure has yielded fewer complications than wrist or intercarpal arthrodesis, and should be considered when both symptomatic radiocarpal and intercarpal arthritis are present.
