Mobile-bearing total ankle arthroplasty (TAA) is a valid treatment option for the rheumatoid ankle if proper indications are used. Aseptic loosening and persistent deformity are the most important modes of failure. Unconstrained TAA with the use of a three-component, mobile-bearing prosthesis has distinct mechanical and kinematical advantages as there is full congruency of the articulating surfaces without restriction of rotational motion. Because of the technical difficulties that can be encountered during surgery, TAA should be restricted to the experienced ankle surgeon.
